What accommodations are available in the area?

There is a large selection of restaurants and hotels located on State Street/Business 20 all within 1 mile of the facility. For more information on local accommodations, please contact our facility.

Where are you located?

The facility is located just east of the intersection of Guilford Road and Mulford Road on Featherstone Road. For detailed directions, please view our map.

How long will my procedure take?

The length of your stay depends on the procedure that is performed. The average stay at our facility is 2 to 3 hours for most visits.

Which insurance plans does RASC accept?

The Center accepts most insurance carrier’s plans, workers’ compensation plans and Medicare. There are several insurance carriers that are contracted with Rockford Ambulatory Center. Please call your insurance provider if you have questions about your coverage.

How do I make an appointment?

Your surgery will be scheduled through the surgeon’s office. You must see a surgeon to obtain an appointment for surgery. If you do not have a surgeon, refer to the list provided of our Medical Staff.

How do I arrange Payment?

As a courtesy, we will submit your insurance claim. The remaining balance of any account is due in 90 days. It is very important that we have correct billing information. We also accept cash, cashier’s checks, Visa, Master Card, American Express and CareCredit. We can accept credit card payments by telephone, with the exception of CareCredit. CareCredit payments must be made in person. Refer to our financial page for additional information.

Whom do I contact if I have a problem with my facility bill?

Our patient account representatives will assist you with any questions regarding your statement of charges. Contact them by calling 226-3300.

How do I request my medical records?

If you would like to request your medical records, please download the Authorization for Release of Information. Complete the form and fax or mail to our Medical Record Department:
